There was something that really bugged me about Byakuya's final decision during SS-arc, when he decided to save Rukia after all. How could he have chosen to fulfill one promise and go against the other? Whether to keep the promise he made to Hisana or the promise he made to his parents(graves). Whether to keep protecting Rukia or to keep abiding the laws of Soul Society? At first he decided Rukia had to die, impliying he chose to keep the promise he made to his parents. Eventually he obviously decided to save Rukia. Well, since that’s the case, you'd think that he then honored Hisana's wish (and went against promise to his parents). That's what I thought at the time, but it didn't seem right to me. He told Rukia he spent all this time struggling about the decision. (EDIT: 23-08: I'm not finished yet!)
